APPLICATION REQUIREMENTS

Once an applicant meets all stated criteria, he/she must submit the following items before meeting with the Chapter Membership Chair for an interview.
  • A fully completed & signed Chapter Application Form. Click here to download.
  • A fully completed Revenue Verification Form signed by the applicant's corporate accountant. Click here to download.
  • A payment in the amount required. Click here to view our current fee schedule.
  • A digital headshot photograph.
